By completing the following form you are entering into a legally binding contract to engage Pete Hartley to perform at your event.

If you have already contacted Pete Hartley and he has asked you to complete the form, please do so with as much detail as possible including the agreed Date, Times and Cost, please take extra care in ensuring the day and date are correct.

Once you have completed the details you will see the contract and be prompted to digitally sign. At this point you will need to make a 50% payment of the agreed amount to confirm the booking contract.

To Set Up Booking Contract Complete the Following Form:

Enter your full name in CAPITALS.

Enter your full address with post code.

Describe the Event such as Wedding, Anniversary, Party etc.

Enter the name or names of who the event is for; such as name of Bride and Groom.

Enter the address of the venue including post code.

Enter date of event - ensure this is correct. Please use this format, Saturday 19th May 2025 (day, date, month, year)

Describe what sections of the event entertainment needed for: Wedding Breakfast, Drinks Reception, Ceremony, Evening Entertainment, All, etc.

Start time.

Finish time.

Enter the agreed amount. DO NOT put any symbols in here just the agreed amount in the format of 0.00. Price entered is in GB pounds sterling (£).
